What insurances do you accept?

We participate in traditional Medicare and all Medicare HMOs, including WellCare, United Health Care, Security Horizon Plan, Cigna Medicare and Blue Cross Medicare. In addition we participate in all Aetna insurance plans, all Blue Cross/Blue Shield plans, Connecticare, Cigna, United Health Care, Husky plans for 21 and under and all private health insurance plans unless otherwise excluded.

What paperwork should I bring?

Please be sure to bring your insurance card, referral if required by your insurer and Worker’s Comp authorization as well as any additional paperwork. We also do require patient forms which are available for download and you can fill these out prior to your visit. There are two patient information forms.
